Question: DS-11 Form section 10 on the passport application form requires information about my parents. I really do not have all of that information. Sadly they past and while I know their names, I have no idea when they were born or where they were born. What do I do? Should I leave it blank?
Answer: It is essential that you fill out as much of the information as possible. The more complete the application the faster it will process and the less likely it will have a delay. If you do not have full information you can provide a rough guess at the information. If you do so be sure to include a question mark after the answer. For example: June 21, 1947? In this way the bureau will know this was an estimation guess and they will be less likely to cause a long delay.
The usual time it takes to process a passport application is 4 – 6 weeks. There is also a expedited passport service that is available for faster processing times. An expedited service will process a passport application in 2 – 3 weeks.
Also, you will be able to contact the National Passport Information Center to check on the status of your passport after 7 – 10 business days.
